Initiative Overview

The Current Gap

Healthcare workers continue to experience high rates of stress, burnout, trauma, and moral injury, which were intensified during and after the COVID-19 pandemic.

Despite this need, many workers face barriers to accessing timely mental health support, including stigma, shiftwork, limited availability of services, and lack of culturally safe, accessible, and confidential resources.

Organizations also often lack scalable systems to provide peer support and early intervention.

Existing supports may not adequately address digital accessibility, organizational integration, or the need for flexible, on-demand mental health resources tailored specifically to healthcare workers.

Beyond Silence was built on over a decade of research at McMaster University and guided by the Consolidated Framework for Implementation Research (CFIR) to ensure evidence-informed implementation across diverse healthcare settings.

The initiative emphasized equity, accessibility, bilingual support, organizational customization, and sustainability.

Findings demonstrated that successful implementation requires leadership buy-in, dedicated organizational resources, proactive communication, and strategies to improve mental health literacy and engagement over time.

Resources associated with the app include an evidence-based implementation guides, promotional materials, and peer support training frameworks to support future scaling and long-term impact across Canadian healthcare systems.

Our Solution

Beyond Silence is a co-designed e-mental health and peer support platform developed by and for healthcare workers.

The mobile app provides private, on-demand access to mental health resources, wellness tools, and trained peer supporters through text and call-based support.

Implemented across eight healthcare organizations representing more than 30,000 employees, the initiative combined app-based support with peer support training, organizational implementation strategies, and ongoing evaluation.

Over 120 healthcare workers across Canada were trained as peer supporters, with demonstrated improvements in peer support competencies and engagement in ongoing communities of practice.

Key Learnings

Primary challenges included varying organizational readiness, competing workplace priorities, limited staff time, and sustaining employee engagement over time.

Differences in mental health literacy, digital literacy, and openness to app-based peer support also influenced adoption.

Organizations also required dedicated leadership support, communication strategies, and internal infrastructure to successfully integrate Beyond Silence into workplace processes and normalize peer support utilization within healthcare environments.
Strong leadership buy-in, organizational alignment, and dedicated implementation resources are essential for successful adoption.

Ongoing communication, trust-building, and normalization of mental health support significantly improve engagement.

Tailored onboarding, in-person launches, educational materials, and continuous reminders help address digital literacy and awareness barriers.

Organizations should also prioritize flexibility, including peer support availability during shiftwork and non-traditional hours.

Continuous evaluation and customization are critical to sustaining engagement and ensuring the platform meets organization-specific needs.

Embedding mental health supports into workplace culture and workflow greatly improves long-term uptake and sustainability.
Collaboration between researchers, healthcare organizations, peer supporters, frontline healthcare workers, and international partners was essential to co-designing and implementing a platform responsive to real-world workforce needs.

Working across diverse organizations enabled testing in urban, rural, bilingual, and community-based healthcare settings while strengthening implementation strategies, app customization, and knowledge exchange on peer support, organizational culture, and digital mental health adoption.

Collaboration further strengthened methodological rigour, implementation science approaches, privacy considerations, and sustainability planning through cross-context learning. Funding for research and development was provided by CIHR, Veterans Affairs Canada, and the Public Health Agency of Canada.

Looking Ahead

Future plans include promoting Beyond Silence to additional healthcare organizations, and creating a sustainability model that optimizes access outside of a research context.

Longitudinal research will continue to evaluate implementation outcomes, utilization patterns, and the effectiveness of digital peer support and early intervention strategies for healthcare workers experiencing stress, trauma, and burnout.
Beyond Silence welcomes collaboration with healthcare organizations, researchers, mental health leaders, and workplace wellness teams interested in implementing scalable digital mental health and peer support solutions for healthcare workers.

Opportunities include implementation partnerships, evaluation research, and knowledge mobilization initiatives.

Collaborators can work with the Beyond Silence team to adapt supports to local workforce needs, improve organizational mental health strategies, and strengthen healthcare worker well-being initiatives.
