We were all gathered around the machine, the newest technology in our office. We’d just been notified that a person from our region who we all knew had just been discharged from hospital and was being driven back to our city (their home). Our Medical Director was quite upset… there was no discharge paperwork in the mail. How could we offer effective community treatment without any discharge information?
